Blake V. Fausett MD, PhD

Oculofacial Plastic Surgeon

Dr. Fausett is a graduate of the University of Michigan Medical School, where he completed his Ophthalmology residency, followed by a two-year fellowship with the American Society of Ophthalmic Plastic and Reconstructive Surgery at the Cincinnati Eye Institute. Dr. Fausett finds that meticulous attention to detail, while still focusing on the broad view, produces the best functional and cosmetic results. Dr. Fausett has the skills and expertise to treat adults and children with various oculofacial conditions.

He enjoys exploring the Oregon Coast, football season, skiing, gardening, traveling, and being with his wife, their five children, and darling grandkids in his spare time. Read more about Dr. Fausett.

Allison Ward, PA-C

Physician Assistant

Allison completed her undergraduate education at Utah State University and trained as a Physician Assistant at Pacific University in Oregon. Before her PA career, she was a physical therapist assistant for 12 years. Before joining Face & Eye Plastic Surgery, Allison gained valuable experience practicing in the fields of gastroenterology and interventional radiology.

Allison enjoys living in Beautiful Oregon. She enjoys adventures with her family to the nearby coast, lakes, and mountains in her spare time. She has tried something new while in Oregon: surfing; next on the list is mountain biking!

Jessica

Lead Nurse / Office Manager

Jessica has been an integral part of Aesthetic Surgical Arts since 2007, serving as both the lead nurse and office manager. With her extensive knowledge of Dr. Fausett's procedures, Jessica is dedicated to helping patients feel at ease and informed about their options.

Outside of work, Jessica enjoys spending time outdoors and cherishing moments with her family and friends. Her passion for her work and commitment to providing exceptional patient experiences make her a valuable assets our team.

Cheryl

Lead Licensed Aesthetician

Cheryl joined our team in 1999 and has been performing spa services ever since. Her clients love her and strive to put people at ease while helping them learn to take care of their skin and the investment they have made in themselves.

Cheryl enjoys spending time with her friends and family, especially her children, Alex, Kinsey, and Katie.

Ashley

Bookkeeper

Ashley joined our team in 2013. Today, Ashley is our bookkeeper, but started at the front desk!

In her free time, she is a certified Lagree Trainer at Lagree Plus in Springfield. She loves to make people feel strong and confident no matter where they are on their fitness journey. Ashley is a boy mama and she loves spending time outside with her family as much as possible.

Jessica

Certified Nursing Assistant and Surgical Coordinator

Jessica has been with our team since 2003. She is our Clinic Supervisor and has over 20 years of experience as a Medical Assistant. Jessica is a wealth of knowledge and always has a smile on her face.

When she’s not working, Jessica is busy raising her two children and two dogs with her husband, Ryan. In her spare time, she enjoys family time spent outdoors or a day in the kitchen baking.

Stacie

Administrative Assistant

Stacie joined our team in 2005 as a front-office assistant. Today, she is the Administrative Assistant for Dr. Fausett’s clinic. She assists with the workflow for both the clinic and the front office. She also supports the insurance and billing office.

Stacie enjoys outdoor adventures and spending time with her family, friends, and puppy.

Karen

Billing Office Coordinator

Karen, her husband David, and their two old English Bulldoggies love to travel/camp in their RV. They are all-season campers and often take advantage of their remote jobs to hit the road and change their environment. Karen’s flexible schedule allows her to help babysit some of their ten grandkids and have an 11th on the way! Karen has done medical billing for 15+ years.

As medical practice and reimbursement are ever-changing, we are fortunate to have someone as conscientious as Karen. Her hard work and determination to get the maximum refund is priceless. Karen spends her free time with her husband, kids, and grandkids.

Shawna

Surgery Center Administration & Certified Medical Assistant

Shawna is our Surgery Center Administrator and a Certified Medical Assistant. She obtained certification from LCC and gained her first work experience in 2014 as an optometry tech in Texas. She furthered her expertise in Ophthalmology after moving back to Oregon in 2015. She enjoys spending time with family & friends.

Heidi

Marketing & Event Coordinator

Heidi moved to Eugene with her husband, Dr. Fausett, in 2016. Before joining Face & Eye Plastic Surgery, she owned and operated a dance studio for 17 years while raising five children and supporting her husband through medical training. Since 2018, Heidi has led event planning, marketing, and social media for the practice.

Outside the office, she loves escaping to the beautiful Oregon Coast with family, gardening, thrift shopping, and finding a great deal—which may explain why Face & Eye is known for fun events, client-favorite specials and the occasional flower bouquet in the summer.

Michelle

Registered Nurse

Michelle joined our team in 2018 and is a registered nurse. She works with both our surgery and infusion patients. Michelle strives to provide our patients with optimal care and make them feel comfortable in an unfamiliar environment.

She enjoys reading, camping, and spending time with her friends and family in her free time. Go Ducks!

Tracee

Insurance Coordinator

Tracee started at our clinic with over five 5 years of experience as a medical receptionist. After spending a year being one of the faces at the front desk, she is now the insurance coordinator for the practice. Tracee helps confirm coverage with insurance companies, determines what insurance benefits are available, and reviews benefits with our patients.

Outside the office, she enjoys spending time with her family and dogs, doing yoga, and attending concerts or sporting events.

Rachael

Referral Coordinator

Rachael came to our clinic in 2019 as part of our front desk team, but today she works in the back office as an Insurance Referral Coordinator.

In her free time, Rachael dedicates her time to her husband, daughter, and dog, Reggie. Rachael loves being with family and friends, cooking, traveling to warm places, camping, and watching football (Go Ducks!)

Liberty

Advanced Aesthetician and Certified Laser Technician

Liberty has been with our team since the Spring of 2019. She knows the value of quality and effective skincare, as her passion for skin began when she struggled with hyperpigmentation and adult acne. She earned her education in Advanced Esthetics and Laser in Arizona but has been in the beauty industry and helping people love the skin they’re in for the last 20 years.

Liberty likes to spend her time outdoors, hiking, biking, and camping. She loves spending time with friends and family, her dog Mia, and cheering on the Oregon Ducks!

Daysha

Aesthetician and Receptionist

Daysha joined Face & Eye Plastic Surgery in the summer of 2021. She has been passionate about skincare for as long as she can remember. As an esthetician, her favorite part is helping people feel confident in their own skin and offering a chance to unplug from their everyday life in a safe and relaxing space. Licensed in esthetics since 2018, Daysha specializes in facial massage, brow shaping, and body waxing.

Outside of work, she is happiest exploring the outdoors with her loyal 10-year-old dog, Milee. Together, they enjoy camping, snowboarding, live music, and any adventure that gets them outside.

Baily

Medical Assistant

Baily was born and raised in Oregon. She became a Medical Assistant medical assistant in 2019 and joined the practice in 2021.

In her spare time, you can find her with her family, friends, and dogs. She loves to spend time outside on the boat, at the coast, kayaking, and camping.

Brooklyn

Front Desk

Brooklyn has been with the team since 2024. She is the first smile you see when you walk through the door at Face and Eye Plastic Surgery. As a gal of many talents, she also helps plan and execute our outstanding events.

When she is not at work, she is busy with her boyfriend, their 2 Golden Retrievers and Chickens!

Karen

Medical Asstant

Karen was born and raised in the midwest, but has always considered Oregon her home. She has been a medical assistant for over 11 yrs working primarily in surgery and orthopedics. She also has a bachelor's degree in psychology.

Outside of work she enjoys spending time with friends and family, traveling, reading, and restoring furniture.

Amy

Front Desk

Amy joined our Front Desk team in 2025. She is one of the voices you may hear when you call our office, and you may see her helping patients at our front desk. She has worked in a variety of medical fields, and is happy to apply her knowledge to help our patients navigate scheduling and with appointment details.

When she is not working, she enjoys spending time with her adult children, friends and family and making melt and pour soap.
